This form is intended to be used by Oklahoma-based organizations to request Adult or Youth Mental Health First Aid trainings for their organizations. There must be a minimum of 10 trainees to book a private training.

Completing this form does not guarantee availability for any dates you provide, please schedule as early as possible to ensure availability on our training calendar.

Mental Health First Aid Oklahoma is a prevention program within DCCCA’s spectrum of Community Based Services. MHFA Oklahoma is funded in part by the Oklahoma Department of Mental Health and Substance Abuse Services and Substance Abuse and Mental Health Services Administration.
